- 1 Hour In-Person Visit - at the request of client or family. Includes Premium Tiered
- Assess Clients Immediate Needs - Share with client or family members.
Bronze Tiered $64.95
- 2 Hour In-Person Visit - at the request of client or family. Includes Premium Tiered
- Assess Clients Immediate Needs - Share with client or family members.
- Coordinate Monthly Appointments - Verify appointment date, time, location, doctor's name, and valid insurance card.
Silver Tiered $99.95
- 4 Hour In-Person Visit - at the request of client or family. Includes Bronze Tiered with 2 additional hours
- Secure Transportation - Client's car, Uber, Medical Transportation, Able Ride, Project Independence.
- Outpatient Services - Coordinate with discharge nurse, family and home care agency
- Outpatient Surgery, also known as ambulatory surgery, or any surgery that does not require an overnight hospital stay.

- Food security, assessing dietary needs. Do the current meals match the dietary needs, for example low or no sugar? Verify how food is purchased and delivered to the client.
- Establish what are the challenges the client is facing as a barrier to eating the number of meals required per day for their health.
- Discovery, what are the challenges the client is facing that cause food insecurities and create a plan to ensure meals are available daily.
- Home safety, walkthrough home safety checklist
- Access daily essential activities both ADL & IADL
- Social well-being, circle of friends, senior center activities, churches/spiritual homes Sunday services, weekday prayer gatherings, and Library. Supporting clients to schedule and attend social events.
- Support adult caregiver family members with resources for self-care.
Silver Package $1000.00
- Include Bronze Package.
- Home-based Care Coordination, creating a care plan.
- Research social and community services.
- Organize documents needed to apply for social and community services.
- Review health care providers, and health insurance
- Review skilled or non-skilled home care agencies to match your loved one's needs.
- Support home aides in understanding the client's specific needs on the 1st day of care.
- Review technology needs (telephone, landline, cellphone and smartphone, video conference healthcare provider, home medical devices, virtual fitness programs, and wearables to detect medical emergencies.)
- Home-based Care Coordination, creating a care plan.
- Organize documents needed to apply for benefits, social services.
- Research social services, community services.
- Review health care providers, health insurance and benefits
- Review bills or insurance claims, for billing errors and resubmit for correction
- Review skilled or non-skilled home care agencies to match your loved ones needs.
- Support home aides in understanding the client's specific needs on the 1st day of care.
