Planning is crucial for seniors who wish to age-in-place. Many seniors prefer to stay in a safe and familiar environment close to their friends and neighbors. This choice is known to have a positive impact on their health, well-being, happiness, and peace of mind.
According to some studies, it may cost over twice as much for seniors to live in a private room in a nursing home or senior living community in New York State compared to staying at home with a home health aide.
As an age-in-place specialist, I provide outreach, assessment, and supportive care management services to seniors who choose to age at home or in a place of their preference.
My services include regular check-ins, emergency support, assistance with healthcare and hospitalization, as well as coordinating home care. I also assess and help enroll individuals in programs that support seniors in aging at home or in a place of their choice.
In my role, I support you, your family members, and your healthcare provider to create a home care plan. I help you develop and achieve health and recovery plans, as well as other life goals. This includes assisting you in accessing necessary health, behavioral health, and other social services supports, and connecting you to the community resources you need.
The main goal of My Age In Place Advocate is to enhance the health outcomes of seniors with personalized needs through effective care coordination.

Transportation
My Age In Place Advocate Transportation Service provides safe and reliable transportation for individuals who wish to maintain their independence while aging in place. Our dedicated team ensures that clients can access essential appointments, social events, and community activities with ease. We prioritize safety and comfort, offering personalized assistance to help our clients remain active and engaged in their daily lives.
A My Age In Place Advocate (MAIPA) representative will:
- Safely drive you to your destination and make you feel comfortable during the trip.
- Help you prepare for appointments and join you as needed.
- Drive you home and stay with you for as long as you need.
- Provide transportation along with companionship and care so you feel secure and at ease.
- Help you communicate confidently with your healthcare provider.
- Support you during your doctor’s visit to ensure your comfort and well-being.
- Offer safe transportation and a range of senior care services designed for all your needs.
Home Care Coordination
- Home safety checklist

- Review skilled or non-skilled home care agencies to match your loved one's needs
- Support home care aides in understanding the client's specific needs on the 1st day of care
- Review health care providers, health insurance, and benefits
- Review bills or insurance claims, for billing errors and resubmit for correction
- Research social and community services
- Organize documents needed to apply for social and community services
- Review technology needs (telephone, landline, cellphone and smartphone, video conference healthcare provider, home medical devices, virtual fitness programs, and wearables to detect medical emergencies)
